To be honest I could have filled it in myself without starting this thread, but as I've never seen a form like this before I thought I would make sense to get people's opinions first.

The fact that it's gone on for 8 pages shows there are lots of different opinions and that people are interested enough to respond and discuss it, which is presumably a good thing.

So Darren, what would YOU have done? With the number of functions you cover, you must have seen all sorts of forms.

Well I'd never have signed a form which called me a client, rather than service provider.

I would have sent them everything I have, H&S policy, risk assesment, method statement etc etc and put N/A on everything that didn't concern me.
