The short version based on my experience...
There are two reasons this may occur.
1) The content of your email looks spammy. If you put in any l links, currency signs, telephone numbers, email addresses, commercial words (free,offer,discount,etc) there is an increased likelihood of being marked as spam.
2) The email headers. If the sending IP doesn't match the domain/headers then it will likely get flagged.
3) Your mail provider reputation. Some mail providers such as Google have a low spam rating, others like hotmail or chinese servers have higher rankings.
As a demonstration, I can easily send out an email that appears to come from ANY email address. However the email headers would flag it if it didn't match. It has to do with something called rDNS.
