Don't want to turn this into a photography debate but I agree, if it's not a fluke, having the assistant in the photo is a no-no. Ever heard of Photoshop lol?
Alternatively, the 'assistant' could have positioned themselves behind the couple (perhaps crouched down) to get the flash right behind them, this, used with fill-in flash either on or off camera would look stunning.

Or they could have used a tripod and taken it when it was as you say when in the right position.

like this: Less energy as people are still sitting down however which is not good at all.
