    Who does them, what do you put in and do you think its worth it?

    Not if I can help it. I would if I was installing quad trussing and overhead lighting etc. But for my little humble set up that does not intrude anywhere near the dance floor (or anywhere with public/guest access) I don't see the point, other than to satisfy insurance companies. I had to do one for a foot ball club once - I was set up right next to the door. To this day it remains the least risky and easy set up i have done.

    its a NAD think really - they want assements from 5 gigs but many of the ones I do wouldn't have any real hazards above what we would normally expect.

    Just wondering if its really worth it

    For risks im looking at

    Slipping, Tripping, Electric shock, Head bangs, Loading and lifting, collisions with equipment.

    can you think of any more

    Hazardous sound levels.
    Temporary blindness(do not look directly into my gigabar units)
    Epilepsy
    Slip hazards (spilled fluid or bubble/foam machines)
    Working at heights

    Nausea(oh no not the Cha Cha Slide again)
    Violence (who let them have a microphone?)...

    I had to do one several years ago to become a regular supplier at a Moat House hotel. It was 24 pages long and had questions like:

    Who does your safety training
    Who is appointed as your nominated first aider
    Who is responsible for arranging electrical testing (PAT testing)

    The number of times I wrote "me", I lost count.

    Initially, I was asked to complete this document everytime i worked there. Ball cocks was the work I used (nearly). I've never been asked for it again.

    However, you should do basic risk assessments as the norm really. Any mitigation of any risk would stand in good stead should the unfortunate happen.
