Anyone know where i can grab one or do you all do your own? This would be the one I send paper form to a customer along with the T & C's to be signed.

Hi Mark,

Personally, I think it's best to do your own as a word document.

That's what I do.

The advantage is that it's personalized...so you can design it how you like & include everything you want.

I used to use these emailmeform (google it) but now I get my web designer do mine.

I will be sorting out a new one soon for my website. Needs changing really.

Just do a google search for Disco terms and conditions and see what comes up.
