After pulling together my T's & C's, I'm now knocking up my booking form, just want to make sure I've thought of everything.
So far I've considered
Client/Booker Name
Address
Contact details
Venue
Address
Contact details
Date
Times reqd
Event
Event Held for.... (Birthday Boy/B&G names)
Special Requirements
Event Running Order
Any specific genres/tracks required

What am I missing...?

Hiya Womble,
I think that covers most of it.
The only other things my own booking form has are approximate number of guests, total cost, deposit, balance and whether a daytime set up is required.
I also have a box entitled "additional information / instructions" where clients can provide any other information they think might be relevant.
Obviously there are spaces at the bottom where they sign, print their name and date the booking form (under a statement confirming they have read, understood & accepted the terms & conditions).

Looks pretty good KJ, although I don't like the word adequate to describe your sound & lighting. To me, and possibly others as well, adequate means "just enough"
Personally, I would change "adequate" to "suitable"
