thats just about the same as what I use. :)

even if you dont use it, or in many cases i leave it lying around on a table whilst i'm setting up - often the bar person/manager or band member will see it and either ignore it or start asking you questions.

About the only places were I actually fill one out are for council owned venues and they expect it.

always worth having in your folder tho - just in case you turn up at a venue and altho its not been mentioned before always surprises them when you pull one out....
