Hi Guys,

I'm looking to send my customers a booking form to write down general information for their upcoming parties, Thing is I don't really know how to set one up as before I have always just asked for their music selection via email, Can anyone on here help me with this or could email me the form they use so I can get some tips from it.

Any obstacles which may mean setup will take longer
occasion
# of guests
any additional extras
rough guideline to music required

deposit
balance
total amount

todays date
terms and conditions
parking restrictions or other useful information
