Information Packs

When I send out info at the moment, I generally send a booking confirmation form, an event questionairre and a self addressed envelope.

However I want to see what the rest of you do as I want to produce an information pack, but am unsure what to add into it.

I have produced a top 100 requested songs for this year which can go in along with a request list (with room for about 15 requests).

So what do you guys use, and care to share?
