I've noticed on alot of the websites that there are online booking forms, how is this done ?

iv copy this from another forum, i use it and it very good and it free.

I've just the other day used this site http://www.emailmeform.com/ to make my new contact form, dead easy to do you get the check boxes you're after and best of all it's free!
