    Default Enquiry Form

I'm in the middle of making a new enquiry/request for what I send out with my contracts. The one I have done looks pants. You know the type of form I mean, the client ticks what type of music they want 60s, 70s, s etc which make the dj's night slightly easier.... Know it sounds cheeky but has anyone got a decent one so I could take some ideas off it.

At the moment mine goes back to my homepage, I think there is a way of doing a confirmation window but haven't worked out how yet!
